Fundraising: Each FOCUS Missions Participant has the opportunity to fundraise the entire cost of his or her mission trip. After you have been accepted on a mission trip, you will be contacted by your personal fundraising coach.

Promoting Safety & Mitigating Risk

FOCUS Missions holds safety for our participants as the primary concern for any mission trip we send. We offer trips to locations once they have been initially researched and vetted with people on the ground, as well as checked and monitored for safety from our security consultant.

Other precautions FOCUS Missions takes include consulting government agencies, such as the U.S. State Department, the Smart Traveler Enrollment Program (STEP), and the Center for Disease Control regarding traveler health. FOCUS Missions also has a number of resources dedicated to promoting safety and mitigating risk, including training every Mission Director in safety and risk management at our Mission Director Leadership Summit, arranging insurance policies for each trip, and equipping our staff and students with safety guidelines at trip orientation.

FOCUS Missions takes great pride in partnering with local religious orders, apostolates, and organizations in every location we serve. We regularly communicate with our contacts before the mission to plan the trip and assess on-the-ground conditions. We reserve the right to re-route or cancel a trip if there is a confirmed safety concern.

Jeff Runyan is a 15-year veteran of FOCUS and currently serves as the Sr. Director of International Relations. Jeff has degrees in Spanish and Political Science and a certification in Latin American Studies from Colorado State University. As a Rotary Ambassadorial Scholar, he studied in a post-graduate program in International Relations at the Pontifical Catholic University in Santiago, Chile. Jeff is also a Rotary World Peace Fellow and has received a certification in International Peace and Conflict Resolution from Chulalongkorn University in Bangkok, Thailand. Jeff served as a missionary at the United States Air Force Academy and spent 10 years as the Director of FOCUS Missions.

Currently we offer short term mission trips that last 10-15 days. You can sign up either as a SHORT TERM TEAM or OPEN SHORT TERM TEAM (specific dates).

Mission Trip Information

Going on mission is an exciting God adventure, but it can come with a lot of questions and concerns about going to an underdeveloped country and the people that you will encounter. Below is information that will probably answer most of your questions about your potential trip. We try to keep our information as up to date as possible. Most of this information can be found in our handbook. The handbook has all forms that need to be turned in prior to the trip, gives an introduction to Hope Ministries Uganda, the country, the Batwa, what to bring. We also have a team leader handbook that helps with organizing your trip.

Mission Teams from Hope Ministries Uganda USA

2017 was a year of great excitement as hope ministries uganda usa brought over their first two mission teams. some of their work is highlighted below., july 2017 discovery mission team.

The first Discovery Mission Team came in July. They had a team of nine from the USA and seven from Kampala. The team ministered to the hearts of the people and lives were transformed because of the work they did. These are just some of the things they did and we praise God for this team and all who partnered with them to make this happen.

  • The Mukungu community was trained on the importance of clean water and taught them how to use Sawyer Water Filters that were donated through Wine to Water
  • Our HDC pastors received first aid training
  • First aid supplies and medicines were also brought
  • Sewing classes were offered to the women and the craft cooperatives were encouraged to use these skills for design and production
  • The team had an opportunity to speak with Members of Parliament and the Minister of Finance
  • A general health clinic was run by the team
  • Updating pictures were taken for the children’s records
  • Shoes that Grow were handed out to all of the children
  • Many recreational games were brought for the children, including jump ropes, soccer balls, parachutes, and much more

November 2017 Discovery Mission Team

In November Hope Ministries Uganda USA brought over a team of 8 from the USA and 7 from Kampala. This team came over to work on our farm project. The team preached, conducted home visits, prayed over the sick and the lost, and loved on all of the children.  Despite the rains and the road conditions, this team worked so hard!

  • Built two barns, rabbit cages, and purchased animals
  • The first barn was built for meat goats and our pastor in Kasovu was trained on how to take care of the goats
  • The second barn was placed at the Rubuguri campus and was built to house 60 chickens, 5 roosters, and many rabbits
  • The team took our younger children from the Rubuguri Campus and gave them there first hot baths. The children were over joyed as they felt so loved.
  • Some of the team also attended a burial of one of our Batwa men. We thank Hope Ministries Uganda USA for helping to make sure this man had a proper burial.
  • Coats and overalls were also purchased and handed out to the children.

How do Mission Trips work?

Short term teams.

For these trips, you gather a team of 10-15 team members from your church, school, or organization and then partner with us to serve in Uganda.

Open Short Term Teams

If you are interested in coming on mission, but do not have a team of your own, you can sign up for one of our open teams.

Size of Teams

Teams of 10-15 are a great size for a team. Teams can split up into two smaller groups if desired to work in different areas. We can possibly go up to 20 in a group, but this would have to be approved beforehand. Our hope is that your group will have similar ages, job skills and activity levels. This will help make the team more productive.

Lengths of Stay

Short Term Teams generally stay anywhere from 10-15 days, which includes travel. 10 is the minimum. Depending on your flight you typically lose about a day coming. You may also need a rest period for jet lag.

Time of Year

Any time of year is a great time of year to take a trip to Uganda. The weather in the Kisoro District during the day is in the mid 70’s and a night it drops down into the mid 40’s. During rainy season it does sometimes drop to the upper 30’s. The temperatures are lower in this part of Uganda because we are located at higher elevations. Airfare varies greatly throughout the year, however. Generally March/April and Sept/Oct are the cheapest times to travel. Winter is the second cheapest, and summer is the most expensive time to travel. Your specific airfare will depend on the airport you fly out of, the carrier you use, and the flight pattern you choose, but generally it will range between $1100 and $2,000. You will fly into Kigali, Rwanda. You will have to apply for an East African Visa which will allow you to cross over the boarder into Uganda. You can book this, or we can book your tickets for you for a truly all inclusive trip cost.

Estimated Cost

Cost will vary based on what type of team you bring. For instance, if its a medical team, the project supplies may be more expensive. Running a full children’s camp will be more expensive than a light construction trip. In general, though, all in-country expenses generally run at $1500 US dollars per person plus project expenses. This is inclusive of all in country expenses. It does NOT include airfare, your East African Visa fee to enter the country ($100) and any pre-trip vaccinations you choose to get. An exact per person cost will be given to you when you complete the preliminary booking form for your trip.

How much does a uganda mission trip cost, short-term mission teams.

Short-term mission teams are 8–25 people. These trips are 10 days long (including travel days) and focus on providing a hands-on, missional, and evangelical experience. Eager to spread the love of Jesus? Like getting a little uncomfortable? Then, you’ll be awesome serving sacrificially alongside our passionate Ugandan staff.

TRIP COST: $2,000 PER PERSON

The $2,000 covers most things from the time that you arrive in Uganda until you depart. Items NOT covered include:

  • Airfare (Can be booked by Okoa US Team or independently)
  • Personal Items
  • Entry Visa (Must be applied for online and received beforehand)
  • Vaccinations (Yellow Fever is REQUIRED. Others are optional but recommended.)
  • Snacks, sodas, or any food other than what is provided at mealtimes

Vision Teams

Vision teams are 3–12 people. These trips are 7 days long (including travel days) and focus on interactive experiences specifically with the ministry areas of Okoa Refuge, providing a glimpse of how the organization functions on the ground. These trips are fantastic for pastors, business owners, corporate partners, doctors, and current or potential partners.

TRIP COST: $1,000 PER PERSON

Types of trips.

Short term trips: For groups, families & individuals , – We can plan a customized team schedule of 1-2 weeks where you can serve a alongside a local church or a non-profit organisation, you will be able to use your passion, talent , profession and gift to serve the local people and empower them meet their needs.

Long-term and short term Volunteerism and Internship: You can stay in Uganda for at least six months as a volunteer or as a student intern to serve with a church or with anon profit organisation, this will give you more exposure, develop your career and make relationships for a life time.

POSSIBLE ACTIVITIES

We want to enable you and your team to operate within your strengths, passion and talent and use whatever creative resources might be at your disposal with a target of meeting the most pressing needs of the host organisation or Church and to also provide you with an amazing outreach experience. NOTE :   for all the activities, we recommend and ensure that a mission team serves with and a long side the local people, so that the local people can own and appreciate a project left behind by the visiting team, this may involve follow up, going into the villages to visit, do trainings, distribute resources, construction and repairs and much more

Once your travel and mission interest information based on the needs of the host church and community have been discussed, we will begin planning a basic Mission trip itinerary. Please keep in mind that this will only be a basic, tentative itinerary. Plans can often change at the last minute and we want to keep our hearts open and obedient to what God is doing

Where We Go

You will see a wide range of situations: villages that have received support for a few years and others that have never been visited by a team.

Depending on team composition, villages visited may include the following villages:

Hotels & Meals

Our mission teams stay in hotels each night. This provides an opportunity for the team to reflect on their experiences during the day, discuss plans for the next day, and connect with each other.

Most often, we stay at the Sunset Hotel International in Jinja. This hotel is situated on the banks of the Nile and is our home away from home when in Uganda.

  • Breakfast - Included with hotel
  • Lunch - Served in the villages and/or snacks on the bus
  • Dinner - Restaurant at the hotel - menu offers Indian, Chinese, Ugandan, and American

All hotel and meal money will be collected prior to the trip.

Finance Information

The cost of the mission trip to Uganda is $2,100 + Air and is all inclusive. Samaritan’s Feet books all flights to Uganda. We arrange ground transportation, meals, accommodations, exit taxes, trip insurance, shoe supplies, etc., which are included in the base cost. We make the process very easy to take a short-term mission trip to Uganda! The only additional funds needed will be for souvenirs and your Visa. An application and $150 deposit are due to secure your spot. All payments can be made directly to Samaritan’s Feet.

Statements will be sent prior to each due date: Application Submission: $150 deposit due June 12: $1,050 due July 12: $1,050 due August 12: Remaining balance due

International & Team Information

You must secure your Visa online for travel to Uganda. This fee, $50, is not included in the trip cost. Yellow Fever immunizations are required for travel to Uganda. Immunizations recommended for travel to Uganda include up-to-date routine vaccinations, Hepatitis A, Hepatitis B, Malaria, Typhoid, and Tetanus. Visit your local Passport Health office or find more information at CDC.gov. Samaritan’s Feet provides additional travel information and tips during training sessions.

You must be 16 years or older to travel alone. If you are 14-15 years old you must be accompanied by an adult. This trip is under the leadership of Samaritan’s Feet.

What is the purpose of the Samaritan’s Feet Uganda Mission Trip? The purpose of the mission trip is to provide shoes and other basic necessities to children and families in need in Uganda, while also sharing the message of hope and love. We offer mission trips in locations all over the world – Join us!

What is the duration of the mission trip to Uganda? The Uganda Mission Trip is an 11-day trip.

What are the requirements to participate in the Uganda mission trip? Participants must be at least 14 years old, possess a valid passport, and complete the required application process.

Do I need to have previous mission trip experience to participate? No, previous mission trip experience is not required. However, participants should be prepared for physically demanding work and cultural immersion. If you are unable to join a mission trip we encourage you to donate to support our work.

What is the deadline to apply for the Uganda Mission Trip? The application deadline is typically 60 days prior to the trip start date. However, it is recommended to apply as early as possible as spots fill up quickly.

What will my team be doing? 

Short-term teams will be working directly with local Ugandan churches that are meeting the physical and spiritual needs of their communities but will have broader opportunities for kingdom impact. While teams will have some opportunities to be part of the feeding programs with churches, teams will also be part of evangelism and outreach in the communities, inviting people to come to participate in church services. Teams will also help the local churches host events as a way of sharing the gospel and teaching the Bible to youth, children, and their families. For a very reasonable price of around £400 per home, your team can also help build a home for a family in need (many teams will choose to help build three homes over the week, but this is optional).

During a trip to Mission Uganda, you’ll get to see and participate in these areas of our ministry: Partner Schools – Visiting some of our partner schools is a highlight for many people. You’ll break a sweat and you’ll get to see what education looks like in Uganda.

In Uganda, 16 women die every day in childbirth. In 2016, there was a doctor to patient ratio of 1 doctor to 24,000 patients. Hygiene in public hospitals is among the worst in Africa. There is no free medical or dental care for the poor, in a country where 20% of the people live below the poverty line and many, especially in the north, live without toilets of any kind. One-third of children under five years old are stunted. Undernutrition is an underlying cause of 60 percent of deaths for children under five and micronutrient deficiencies, including in vitamin A and iron, are highly prevalent in women and children. Uganda’s population is the second youngest in the world, with half the country’s population younger than fifteen years old. The country has grown by more than 10 million people in the past ten years. Four out of five Ugandan youth (21% of the total population) are unemployed–the highest rate in Africa. For all of these reasons and more, Uganda is a country greatly in need of the acute medical and dental care and community health education provided by IMR. Malaria, respiratory infections, and diarrheal disease are among the most common causes of premature death in Uganda. You will play an important role in the treatment and prevention of these common infections.

A 2016 study found that there is only one physician for every 24,000 patients. Needless to say, the work that you and the IMR team will perform in Uganda will make a lasting difference on the lives of your patients.

During clinic, you will help to treat conditions related to common infections, malaria, respiratory conditions, dihedral diseases, HIV/AIDS, and tuberculosis. Since many diseases are treatable, your team will administer vaccinations and provide education on how to remain healthy. Another main focus of this mission is dentistry. Because all community members are encouraged to attend, the demand for dental care will be high.

International Medical Relief began sending medical mission trips to Uganda in 2013. Since then, we have deployed nearly 150 IMR volunteers and staff and served more than 12,500 patients. We will continue our efforts to serve Uganda, working alongside local officials to provide medical and educational supplies to reach our goal of delivering sustainable healthcare to the community.

With diligent planning and preparation, IMR is dedicated to ensuring that specific goals are met on our missions, each in correlation to the United Nations Sustainable Development Goals 3 and 17.

IMR works to elevate vulnerable populations through shoulder-to-shoulder care with our local partners, creating workforce density, and a cadre of future healthcare and public health leaders in association to UNSDG 3: Good Health & Wellbeing. 

In correlation with UNSDG 17: Partnerships for the Goals, our organization works in collaboration and at the request of local hosts where we are under their direct influence and affluence. IMR never goes into a country without a formal invitation where there is a great community need for medical or dental care, and health education.

Your team will fly into Entebbe and transfer to a nearby hotel where you will spend your first night. From day two onward, you will be accommodated in Jinja in a clean, comfortable but modest hotel. Your team will have the opportunity to explore many small villages while providing care. A village stay for one night may also on the agenda!

Trip Highlights

In clinic, you will find that your efforts make a tangible difference as you treat many patients with infectious disease, large wounds, and a great deal of mosquito and water-borne illness. You’ll be able to do house calls for homebound elders, provide acute care for the large Albino community in the surrounding area, and work in a village that will welcome your care with open arms and giant hearts.

After a busy clinic schedule, the team will see some of the world’s most amazing animals “up close” on your one-day safari, which is included in this trip! Uganda is an excellent place to spot the “big five”–the African lion, African elephant, Cape buffalo, African leopard, and rhinoceros! But that’s not all, your team will also have the chance to visit Lake Victoria and the infamous equator line which separates the southern and northern hemispheres.

Food & Water

All food is typically provided during your stay. Please refer to your unique itinerary on our website to review which meals are provided each day. We may hire local people to cook for the team during our visit, or we may eat at restaurants or hotels. Lunch in clinic may include sandwiches made by the team or a feast prepared in your honor by community leaders. We recommend bringing snacks to eat during the day or evening or to supplement your lunch. High protein/high fat/high energy snacks are best. IMR also provides all of the drinking water for the team. Hydration and nutrition are extremely important for the health of the team. You are the best judge of your own health so please pack appropriate snacks and stay well hydrated on the trip.

Full Mission (7-8 Days)

You and the team will fly into Entebbe International Airport (EBB). Upon arrival, you will transfer to your hotel where you can check into your rooms and enjoy a welcome dinner at the hotel. On the way, you will stop by Lake Victoria to snap a few photos of the magnificent sights! Upon arrival in Jinja, you will attend the team orientation to prepare you for the upcoming week of clinic. Your team leader will assign roles for clinic, address IMR’s clinic protocols, and discuss the communities you will be serving in Uganda.

Education is as important as medical and dental care for our patients. Each clinic includes basic health education according to the specialties and skills of the IMR volunteers, as well as the needs of the local people. When communities become empowered to take health and wellness issues into their own hands, they become self-sufficient. IMR provides learning opportunities for the local communities through classes and hands-on learning to sustain their well-being beyond our visit.

Breakfast will be provided at the hotel each morning. You’ll help pack the IMR supply bags and depart for clinic! At the clinics in Uganda, there is a great need for acute medical and dental care. Malaria, respiratory infections, and diarrheal disease are among the most common causes of premature death in Uganda. IMR clinics play an important role in the treatment and prevention of these common infections.

You’ll also participate in community health education, addressing preventative measures & sustainability.

After clinics, you’ll head back to the hotel for dinner & free time in the evenings. Your last day in clinic will likely find you exhausted after a week of hard work, but also wondering where the time has gone – and when you can come back.

After days of working in clinic, you’ll depart for a day of safari! Although no sighting is promised, your safari will take you a little closer to spotting Africa’s classic “big five” – the African lion, Cape buffalo, African leopard, African elephant, and rhinoceros! After an adventurous safari day, you and the team will travel back to Entebbe for an overnight stay.

Enjoy one last Ugandan breakfast and bid farewell to the beautiful country of Uganda. Airport transportation is provided for all group flyers.

Crisis Management & Emergency Response

We take security very seriously at IMR. It is our job to ensure that security precautions are in place, but please keep in mind that we cannot guarantee your safety on the trip.

  • We continuously review all current activities occurring on the ground, and we work with the State Department and the U.S. Embassy to know the ground situation to the best of our ability.
  • A security training conference call is available to our volunteers where we detail our security procedures that are in place for the trip.
  • We take many security precautions such as radios, a buddy system, emergency evacuation plan and security planning.
  • We protect the privacy of our volunteers in the field and provide them with customized contact information for their use.
  • During our mandatory briefing, team members review our safety protocols, evacuation procedures, protocols for illness, natural disaster and terrorism. We provide training to our teams to be cognizant of their own safety and to be aware of their surroundings.
  • Several policies require teams to adhere to safety issues.

Kumi in Eastern Uganda will give you a taste of the ‘real Africa’. The colours are bright, the welcomes are big, the wildlife is jaw-dropping and the scenery is breath-taking. However, underneath that traditional view, the need is significant. We’ve worked with partners here for many years, supporting the need for additional classrooms and further educational facilities to support the growing class sizes. Education is the key to breaking poverty and we’re working to help children here have access to full-time schooling.

The Projects

Partner – kumi primary school.

Our host in Kumi is the PAG (Pentecostal Assemblies of God) Church.

In 2022 Mission Direct moved to work at Kumi Primary School. This is a State Primary School which was founded by Missionaries at St Stephen’s Church in 1926. Some of the original buildings are still in use together with additional classrooms from 1980’s which is when it was handed over to the Municipality. There are over 1,200 children at the school, and this will double over the next ten years.

In 2022 we built homes for four teachers families. This enabled us to renovate an old classroom block which the teachers have been living in. The result has been to halve class sizes (to around 80) for the first four year groups.

Our task at this school is to continue to provide improved facilities. In 2023 we built a new two classroom block. Our project for 2024 is to build another two-classroom block to continue to increase capacity, reduce class sizes and create improved learning environments.

Rest and Relaxation

At the end of your time in country, you will stay at Red Chilli Camp in Murchison Falls National Park for a wonderful safari and some well-deserved downtime.

What to Expect...

The first thing you need to know about taking a mission trip is that you must be flexible with your expectations and plans, because they can change and probably will.  That said, this page is an overview of what this year's mission trip will be. Check back often as it will be updated as plans get put in place closer to the trip.

Trip Travel Plan:

Fly out of Grand Rapids (or your prospective airport) on January 27th and arrive in Entebbe, Uganda on the 28th.

Uganda Vision Trip

Since 2014, Water Mission has been providing clean, safe water in Northern Uganda – serving the needs of refugees from South Sudan and the Democratic Republic of the Congo. Through our partnerships  with supporters, the U nited  Nation’s   refugee agency, and  the  local government , 230,000 refugees have had access to safe water.  On average, refugees in  Northern  Uganda live in settlements for 10 years or longer. We’ve been meeting  their  long-term needs for safe water, sanitation, and hygiene ,  helping to improve their living conditions   as they rebuild their lives.  

Trip Overview

On this vision trip, Water Mission partners will witness the impact of safe water in the Bidibidi refugee settlements located in the northern region of Uganda and in the rural communities of Jinja. Participants will see existing safe water solutions that provide several communities with reliable, solar-powered sources of safe water — technology and innovation that have earned us a reputation for solar power expertise within the development sector. This trip is an opportunity to see how the provision of safe water supports the health, education, and livelihood of men, women, and children fleeing political conflict and violence. For many of these refugees, access to safe water means they have one less thing to worry about as they re-establish their lives. Along with refugees, our work has been beneficial to rural Ugandan populations who must share their limited resources with people taking refuge in their country. On this trip, Water Mission partners will see how the provision of WASH solutions has blessed both Ugandan natives and their visitors.

The purpose of this trip is for participants to:

– learn about the global water crisis, – gain experiential knowledge of how Water Mission is working, – listen for God’s calling on their involvement in this work, and – be equipped to be an ambassador and advocate, helping Water – Mission achieve its mission of sharing safe water and God’s love.

Trip Itinerary

This 8-day trip includes travel to and from Uganda and 5 days on the ground. The itinerary will be planned around meaningful interactions with Water Mission staff and the people we serve. Your trip will include: – Time spent in the Bidibidi and Omugo refugee settlement, and – communities surrounding the Water Mission office in Jinja – A visit to the Office of the Prime Minister – Devotions with country staff – Cultural experiences in Kampala and Murchison Falls State Park

This trip will cost $1,650 for an individual registrant, and $3,000 for a double registration . This will include accommodations, in-country meals, in-country transportation, and related program costs, all booked by Water Mission. This does not include international flights, immunizations, medications, health insurance, and incidental expenditures. Because the group travels together from a connecting location in the US on one flight to Uganda, we will not begin booking tickets until the registration deadline or when the trip is full. The vision trip coordinator will work closely with you after that date to book your tickets. Payment in full for the trip is required prior to trip departure. We ask you to pay for the trip in two installments. The first payment will include your in-country costs and is payable by credit card at the time of registration. The second payment will be for your flight costs and will be due when the flights are booked. Approximate airfare from Atlanta will be $2,000 per person. The trip coordinator will ask you to approve the final itinerary before booking.

Meeting in Uganda

A midwife and an obstetrician meet at orientation…

Cathy Han, Ariel Bernstein, and Dr. Bameka Agery at the Busoga Health Forum office in Jinja, Uganda

Ariel and I met during an orientation week at Harvard T.H. Chan. She is a midwife from Maine, and I am an obstetrician from California. We were both interested in volunteering in Uganda, a country with a high maternal mortality rate of 284 maternal deaths per 100,000 women, compared to 21 per 100,000 in the United States. I had experience with implementing best practices in hospital settings and she had experience with delivering babies in low-resource areas. We quickly realized that our skills would complement each other.   

In the fall, we partnered with two Ugandan physicians and the Busoga Health Forum , a community organization committed to improving health outcomes in the Busoga region of eastern Uganda. Their mission is “to rally Busoga professionals to use evidence to engage community, government and partner action for better health of children, youth, women and men in Busoga.” By utilizing their Whats App group, they successfully mobilized forty midwives to participate in our simulation trainings that were to take place at Jinja Hospital. We used the funds from our Rose Service Learning Fellowship award to provide transportation, lunch, anatomical models, and training manuals for the midwives.   

On our first day in Jinja , Uganda, we got some curious looks on our way to the hospital. Ariel and I each rode on the back of a motorcycle taxi or “boda boda,” carrying a mannequin in a large backpack. Ariel laughed at how tightly I was gripping onto my driver and told me to lighten up so he could breathe. As we set up our materials in the hospital library, we were relieved as the first group of midwives arrived. In their dresses and heels, they looked like they could be going to church. They told us it was not every day that they had visitors from Harvard ! . They were used to didactic lectures but had never done simulation drills with role playing and mannequins.

Ariel and I e took turns leading the simulation drills and played to our strengths. Ariel had more experience with vaginal breech deliveries and demonstrated the maneuvers with the mother-baby model. I reviewed the protocols for postpartum hemorrhage and pre-eclampsia that my California hospital in California had implemented as “best practices.” We soon discovered that some of the midwives were natural teachers. Sister Betty made us laugh as she dramatically wailed as if she were in labor. Sister Phiona showed us how to improvise an intrauterine balloon device from an inexpensive catheter and a glove.

On the third day, we visited Labor and Delivery and the midwife on duty unexpectedly recruited us to help for a few hours. She handed us gloves and Ariel delivered a baby 30 minutes later. I resuscitated two babies who were having trouble breathing. There were no delivery beds or drapes; women gave birth on top of a rubber mattress covered by a black plastic garbage bag. The unit had one bulb syringe to suction all the babies. One woman had a hemorrhage, but the midwife couldn’t draw up oxytocin because they had run out of syringes. I asked if we could give her tranexamic acid or methergine or a blood transfusion, but none of those were available.   

After we left the hospital, I told Ariel, “I’m so glad you’re here…because it would be hard to explain what this was like to anyone in the U.S.” We had not expected the conditions to be this dire in a large regional hospital. We now had even more respect for the midwives’ ability to practice with such limited resources.   

In California, I’ve delivered 3000 babies over 15 years and have never experienced the tragedy of losing a mother. In Jinja, one of the midwives gave us a tour and pointed out a patient who had been transferred to the hospital after a significant postpartum hemorrhage at home. Sister Joselyn said “she’s going into shock… I’m not sure she’s going to make it…and there’s no blood available.” Ariel and I asked if we should donate blood, but we had the wrong blood type. It felt strange and terrible not to be able to do anything. We asked Sister Joselyn about the patient the next morning and she just shook her head slowly.   

Ariel and I helped each other process the trauma of not being able to save a woman from dying in childbirth. We contemplated the toll it must take on the midwives. Our training seemed insignificant compared to the major challenges they face. What saved us from despair was reading the midwives’ comments on their post-training surveys. They universally reported that they had learned skills and had more confidence in their abilities.    

Ariel and I have thought about how to make the training in Uganda sustainable. Our goal is to return in the summer and train a few of the midwives to become trainers themselves. We felt incredibly lucky to be able to teach as a team and plan to return together. Despite the lack of resources, we hope to empower the midwives with the tools of knowledge and confidence.

USAID Contributions to The 2024 Global Health Security Strategy

Today, the White House  released  the 2024  U.S. Global Health Security Strategy,  reaffirming its commitment to global health security (GHS), recognizing that disease threats emerging anywhere pose risks everywhere .  The Strategy outlines the Administration’s approach to working with partners around the world to prevent, detect, and respond to infectious disease outbreaks faster.  Delivering on this Strategy will protect the health, lives, and livelihood of both the American people and people all over the world against infectious disease threats. 

Incorporating lessons from the COVID-19 pandemic, the Strategy further outlines the U.S. government’s international support for achieving a stronger, more secure global health security architecture. The Strategy also complements other policies focused on preventing, detecting, and responding to infectious disease threats, including the  National Biodefense Strategy ,  Executive Order 13987 ,  National Security Memorandum-1 , and  Executive Order 13747 .  

USAID’s work in support of the Strategy builds on 60 years of strengthening health and economic systems, decades of experience with international outbreak and disaster response, and more than 20 years working with countries around the world to  develop the global, regional, and country-level health security capabilities  needed to prevent, rapidly detect, effectively respond to, and ultimately, recover from emerging infectious disease threats.

USAID’s global health security efforts contribute to each of the three goals of the Strategy: (1) Strengthen Global Health Security Capacities through Bilateral Partnerships; (2) Catalyze Political Commitment, Financing, and Leadership to Achieve Health Security; and (3) Increase Linkages Between Health Security and Complementary Programs to Maximize Impact.

KEY ACTIONS

Goal 1: strengthen global health security capacities through bilateral partnerships.

USAID’s GHS Program directly contributes to the GHSS 2024 whole-of-government commitment to achieve and sustain demonstrated capacity, or a comparable level, in at least five GHS technical areas in 50 partner countries (or regional entities), and to catalyze other partners to support the same goal in 50 additional countries. Since 2022, USAID has significantly expanded its GHS investments to reach  more than 50 countries . 

USAID will continue to work with national authorities and other stakeholders – including non-governmental organizations, U.S. and host-country universities, the private sector, multilateral organizations, research institutions, and various local partners – to address the highest priority gaps countries need to fill to prevent and be prepared for pandemics and other health emergencies. 

Goal 2: Catalyze Political Commitment, Financing, and Leadership to Achieve Health Security

USAID works to garner the necessary political will to end the cycle of crisis and complacency related to international health security and pandemic preparedness, and works with partners to replace this cycle with continuous financial and political support to more effectively assess, prevent, prepare for, respond to, and recover from emerging infectious disease threats, including pandemic threats.

As part of a whole-of-government effort, USAID works with and invests in the  Pandemic Fund , which finances critical investments to strengthen pandemic prevention, preparedness, and response capacities at national, regional, and global levels, with a focus on low- and middle-income countries. USAID also works with and invests in the  Coalition for Epidemic Preparedness Innovations (CEPI) , which accelerates the development of vaccines against emerging infectious diseases and enables access to these vaccines during outbreaks.

Goal 3: Increase Linkages Between Health Security and Complementary Programs to Maximize Impact

USAID’s broader global health and development programs are inextricably linked with GHS investments. USAID leverages its GHS Program to reduce epidemic and pandemic shocks on country health systems and to link national and regional initiatives that build horizontal, non-disease specific capacity – such as safe and secure laboratory and disease surveillance systems, human and animal health workforce, and platforms for procuring, deploying, and managing countermeasures and personnel in response to health emergencies. 

All of USAID’s global health programs contribute essential support to achieving outbreak preparedness and response objectives, including programs supporting health systems resilience; primary health care; maternal and child health (including immunization) and nutrition; the  U.S. President's Emergency Plan for AIDS Relief (PEPFAR) ; the  President’s Malaria Initiative (PMI) ;  COVID-19 preparedness and response ; and  food security .
