Here at EG Healthcare, we want to bring them back for seniors on Staten Island. That’s why we are doing something about it. EG Healthcare’s medical house calls provide patients with medical treatment, the same as in a doctor’s office, in the comfort of their own home. Our house call medical services specialize in primary care, well visits, sick visits, palliative care, and chronic disease management.

According to the National Council on Aging, approximately 80% of older adults have at least one chronic disease ,  and 77% have at least two. What is a Medical House Call?

A house call is similar to a doctor’s office visit; however, in this case, it is provided by a nurse practitioner in a patient’s home.

  • A qualified and experienced medical professional will schedule a time for your visit and arrange a consultation.
  • During the appointment, our medical professionals can create orders for an ongoing medical treatment plan (all in the comfort of your own home) that meets your needs.

We provide primary care, well visits, sick visits, palliative care, and chronic disease management in addition to creating orders for physical therapy, speech therapy, occupational therapy, dietician and nutrition, podiatry, optometry, and more.

For Primary Care, we can, among many other things:

  • Administer vaccinations
  • Provide preventative care such as screenings
  • Check the patient’s vital signs
  • Give a head to toe examination
  • Handle bloodwork
  • Provide an electrocardiogram (EKG),  
  • Administer an eye exam
  • Perform cerumen removal
  • Depression screening
  • Tobacco screening
  • Nutrition assessment & counseling 
  • Provide a fall risk assessment,  
  • Cognitive assessment (mini-mental exam)

We can also assist with Activity of Daily Living (ADL), assistive device ordering, including canes, wheelchairs, walkers, and hospital beds.

For sick visits, we can treat, among other conditions:

  • Urinary tract infections (UTI)s
  • Upper respiratory infections (URI)
  • Foley catheter management
  • Eye and ear conditions

For palliative care, we can treat and assist with:

  • Pain management
  • Advanced Directive/ MOLST Form . Including reviewing with family and patient if they have the capacity. We can help navigate and understand advance directives, including cardiopulmonary resuscitation (CPR) and do-not-resuscitate orders (DNR)
  • Provide hospice and end of life care
  • Help to review a living will

Help treat chronic diseases, such as:

  • Chronic obstructive pulmonary disease (COPD)

Who will my provider be?

EG Healthcare’s House Calls for Seniors providers are Adult-Gerontology Nurse Practitioners. An AGNP is an advanced practice nurse with a professional focus on adults through seniors. In addition to their certification as a registered nurse, an advanced practice nurse has a master’s degree, a post master’s certificate, or a practice-focused doctor of nursing practice degree. 

To become an AGNP, an individual must enroll in either a master’s or doctoral program at an accredited institution. These institutions may have specific tracks and specializations, including acute care and primary care.  

A Nurse Practitioner can:

  • Diagnose and treat acute conditions
  • Order diagnostic tests including x-rays and lab work
  • Manage a patient’s overall care as a primary care provider
  • Write prescriptions

How will a home consultation look like?

Our Nurse Practitioner will arrive at the scheduled time, and they will ask some questions to get to know you better. After getting to know you better, the Nurse Practitioner will conduct the necessary physical exam and, if required, order any needed labs or imaging and prescribe medications. They will then discuss any next steps and schedule follow-up appointments with the nurse practitioner or write orders for any necessary therapies.

Will my parent have the same Nurse Practitioner every time?

We will try to send the same Nurse Practitioner every time, but we cannot guarantee this; it is highly dependant on scheduling.

Are all insurance types accepted?

The patient must have insurance, in order to discuss any questions please feel free to contact us .

Nurse-Family Partnership®

Our Nurse-Family Partnership® (NFP) program is a free home-visiting service that serves first-time moms by providing valuable knowledge and support beginning early in pregnancy and until the child reaches two years of age. Partnering first-time mothers with a personal nurse who will provide services to the family based on their schedule empowers these mothers to confidently create a better life for their children and themselves.

Nearly 40 years of research has consistently proven that the Nurse-Family Partnership® program succeeds at its most important goals — keeping children healthy and safe, and improving the lives of moms and babies.

First-time parents can get free help from the Newborn Home Visiting Program. Health workers will visit your home and can help you with breastfeeding, mental health education and screenings, provide health and safety tips, and connect you with community resources.

Due to COVID-19, in-person services are temporarily suspended. The program is supporting families by phone.

  • Parents receive three home visits and a follow-up phone call.
  • Parents can be eligible regardless of immigration status.

Who is eligible

To be eligible for this program, you should be a first-time parent who lives in one of these neighborhoods .

You may also be eligible if you are a parent with a baby less than 3 months old and who:

  • lives in a NYCHA development in one of these neighborhoods .
  • current gets help from the Administration for Children’s Services (ACS).
  • lives in a Department of Homeless Services (DHS) shelter.

In operation in Staten Island for more than a decade, The Foundling’s Healthy Families program is a free, home visiting program designed to help new and expectant parents meet the challenges of parenting and ensure the healthy development of their children.

Being a new parent can be stressful and overwhelming, and many families, particularly those who are socially or geographically isolated, struggle to provide a supportive environment for their children. The Foundling’s Healthy Families program helps prepare new parents to meet the challenges of parenting and ensure their children’s healthy development.

The Healthy Families program promotes child and family health, development, and self-sufficiency through home visiting services that support parents from pregnancy until their child is ready for school. The program provides support and information to families and  links them to resources in the community, while promoting well-baby care.

EARLY INTERVENTION FOR CHILDREN

The early years of a child's life are very important! VNASI's Early Intervention professionals participate in a statewide program that provides specialized services for children from 0-3 years of age and their families. We work closely with you to create a program tailored for your child.

MATERNAL CHILD HEALTH CARE

VNASI Maternal Child Health Care Program provides prenatal and postpartum care to mothers and infants. This program also offers specialized health care to children up to 18 years of age.

IN-HOME ACUTE CARE SERVICES

VNASI Acute Care Program provides shsort-term skilled nursing services (Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, Nurtritional Services) after hospitalization or on doctor's orders to ensure that patients achieve the highest possible level of recovery and self management in the community.

HIGH TECH THERAPIES

VNASI High Tech Program provides a variety of intravenous, enteral, and inhalation therapies which offer patients a welcome alternatives to hospiatalization or difficult-to-travel-to clinical facilities.

Most home health care services provided by Visiting Nurse Association of Staten Island are reimbursable through private Insurance, Health Maintenance Organizations (HMO), Medicare, or Medicaid. VNASI will be pleased to work with you and determine your eligibility for coverage.

Medical Home Visit Programs During COVID-19 State of Emergency

This report highlights the unique challenges faced by home health programs in addition to discussing how technology and policy changes have helped the elderly homebound during the coronavirus disease 2019 (COVID-19) pandemic.

Elderly, homebound individuals comprise a vulnerable segment of society who have been disproportionately affected by the coronavirus disease 2019 (COVID-19) pandemic through a myriad of unique challenges. There is a significant amount of fear of acquiring COVID-19 by seeking health care services, which has adversely affected patients by worsening fixable situations. Another challenge is the decrease in diagnostic support for evaluating patients compared with a pre–COVID-19 world. Agencies providing at-home phlebotomy, portable radiology, and support services have had to limit their home visits due to an inability to access personal protective equipment. This loss of diagnostic and therapeutic support has had an emotional toll on patients and their caregivers. COVID-19 has had a tremendous impact on the health and finances of home health aides and their patients. Loss of long-term home health aides has adversely affected younger patients with ailments like Down syndrome as well as older patients with dementia. COVID-19 has also increased pressure on end-of-life decision making. Patients and their families are increasingly opting for palliative care and hospice programming to avoid separation. Families are being forced to consider advance directives under an increased emotional strain as patients become “persons under investigation” for COVID-19. Technology has allowed for the provision of services through telehealth, and changes to policy by CMS have aided widespread implementation of telemedicine. We anticipate continuing to be nimble in the face of challenge and to provide timely and meaningful care for those who depend on our efforts.

A vulnerable segment of society is composed of elderly homebound individuals who can no longer care for themselves. We have witnessed bravery by families who embrace their elders with respect and love. Homes are redesigned to incorporate safety and creative design to facilitate accessibility for those with disabilities. Unfortunately, there also are those who suffer neglect by family and social isolation. We care for those who experience financial abuse, poverty, and burdens of chronic disease.

Our medical home visit program is embedded in the Division of Geriatrics at Staten Island University Hospital in New York. We provide ongoing medical visits for 325 families with a professional staff of geriatricians, family practice physicians, a physician assistant, nurse practitioners, a social worker, and a clinical pharmacist. Our office staff facilitates every imaginable need of this population. We have seen our share of natural disasters. While our hospital was being evacuated for Hurricane Irene in 2011, we approached Incident Command with our plan of contacting each family to assess needs. In 2012, while preparing for the impact of Superstorm Sandy, we had some homebound individuals transferred by ambulance to shelters. Staff members purchased and delivered food, and processes were coordinated with oxygen companies, visiting nurse agencies, and Meals on Wheels to predeliver goods. After each disaster, we assisted patients in recovery and safe relocation. The needs emerging from our current emergency are unlike those from any of our past crises. During the coronavirus disease 2019 (COVID-19) pandemic, we have experienced a myriad of unique challenges.

There is an exceptional amount of fear on the part of patients and families to access the health system with concern of coming in contact with COVID-19. This fear impairs good judgment. For instance, a patient taking Coumadin developed black diarrhea and light-headedness yet refused to go to the hospital for decreasing serial measurements of hematocrit. He said, “Hospitals are full of coronavirus and I won’t go there.” There is a fear of isolation because hospitals will not allow visitors, which also leads to poor decision-making. A patient with an acute cerebrovascular accident, with new-onset oropharyngeal dysphagia and the pocketing of food, progressed to fatal aspiration pneumonia because the caregiver could not bear the thought of sending their loved one to the hospital alone.

Another challenge is the decrease in diagnostic support for patient evaluation compared with a pre–COVID-19 world. Vendors have been limiting their home visits, and as a result, our program has had less diagnostic support. Our portable radiology vendor went on hiatus. Previously reliable laboratories for home phlebotomy offered fewer rounds, and patients lost the benefit of urgent testing from home. Testing for COVID-19 by both polymerase chain reaction and antibody identification has been relatively nonexistent for the first few months of this pandemic for the homebound.

The inability to access personal protective equipment (PPE) placed severe limitations on our visiting nurse partners. They were unable to visit patients because they did not have PPE to protect themselves and not all the agencies were prepared to conduct telehealth services. Without these community agencies, support services such as physical therapy, occupational therapy, and speech therapy came to a staggering halt.

Another burden with the loss of diagnostic and therapeutic agencies is the emotional toll of losing the reassurance that these providers gave to patients. Regular contact by nurses and therapists plays an integral role in providing emotional support to overstressed caregivers. Positive reinforcements from these service providers ensure validation for families that they are providing appropriate care. Such reassurance is invaluable in supporting caregiver efforts.

COVID-19 has had a tremendous health and financial impact on home health aides. Some of these dedicated professionals have contracted the disease themselves. While ill and convalescing, they are unable to work, and this has taken a financial toll on this force. Conversely, when a long-serving aide is taken out of rotation for a patient with advanced dementia, there is an increased risk of the patient developing delirium. Continuity of care providers is one key to success in care of adults with dementia. Additionally, there is the risk that aides may bring COVID-19 to patients either by having mild illness or as asymptomatic carriers. Our program has seen this tragically affect younger patients who are compromised by congenital ailments such as Down syndrome and older patients with dementia.

Another impact of COVID-19 on the homebound community is the pressure it brings to end-of-life decision-making. Even in patients who are chronically ill with a high degree of disease burden, some patients and families refuse to address issues relating to assigning someone as a health care proxy, establishing advanced directives such as Medical Orders for Life Sustaining Treatment, and considering palliative care and hospice programming. As patients become “persons under investigation” for COVID-19, their families have been forced to consider these issues under scenarios of increased emotional strain. An increased number of patients have transferred to hospice whose families previously would not consider it. The driving force in most of these situations was that the family did not want to be separated from the patient as they were dying. Most of these cases should have been on hospice already, but pre–COVID-19, there was a significant amount of familial resistance.

Technology has assisted in our provision of services. Families have embraced telehealth and have been very supportive and understanding of the strains on our professional and support staff during this crisis. They express appreciation for telehealth visits and to staff for their assistance in day-to-day operations. Patients have benefited in ways that would not have been earlier imagined. For instance, a patient had fallen and, after being lifted into a chair, was afraid to move for fear of hip fracture. When we were called, we had the family extend her legs with underlying support so we could see whether there was leg shortening or outward rotation of the foot. Upon seeing neither, we were able to coax the patient to ambulate with direct visualization. Other assessments have included shingles, bed bugs, olecranon bursitis, cellulitis, and exacerbations of congestive heart failure and chronic obstructive pulmonary disease—all handled with minimal exposure to the patient and with gratitude from the family. We have trained families to assist in meaningful data collection activities like tracking temperatures, measuring heart rate, and performing pulse oximeter checks so that we could identify patterns of concern. Additionally, we have embarked on a program to have medical students provide “social calls.” They check on how patients are doing, and if a need is identified, including the need for psychological services, then appropriate referrals can be made.

We have been grateful for actions the federal government has taken to help ease transitions from the community to a skilled nursing facility when needed—for example, for the urgent care of a patient with dementia who could no longer be handled at home. CMS rescinded the “3-day rule,” which required a hospitalization for 3 days before Medicare allowed for payments in a skilled nursing facility. 1 Further federal and state allowances to decrease regulation and support payment for telehealth visits have provided needed fiscal support of programs that virtually bring medical professionals into the home to direct care. 2 We anticipate continuing to be nimble in the face of challenge and to provide timely and meaningful care for those who depend on our efforts.

CAMBA’s HomeBase homelessness prevention program serves more than 8,000 clients every year with excellent outcomes . We provide a variety of homelessness prevention tools to assist families and individuals experiencing a housing crisis, and who are at imminent risk of entering the shelter system. HomeBase offers the following services directly or through referrals to our community partners: budgeting and financial workshops, education and employment support, entitlement advocacy, legal services/informal mediation, resources on how to look for housing, and short-term financial assistance.

We serve eligible applicants living in Brooklyn Community Districts 2, 3, 6-15, and 17), or any Community District in Staten Island. Should you know someone experiencing a housing issue in these communities, please feel free to contact us or call 311 to be directed to your closest HomeBase office. CAMBA’s YOU CAN VAN (YCV) is a custom-built recreational vehicle, which serves as a mobile outreach and assessment center for housing stability and intervention services.

The YCV uses real-time data from the Department of Homeless Services and mapping technology from Arachno to map shelter applicants and tenants in Housing Court and target specific buildings and blocks with high numbers of tenants who are at risk of eviction or entering shelter. By bringing our services directly into high-need neighborhoods, we increase visibility, remove barriers to access, and ensure families can receive referrals, advice and material help right when they need it most.

The YCV is fully staffed by our bilingual outreach team, including a supervisor, driver, assessment worker and outreach worker, who set up informational tables and provide onsite assessments and referrals to supportive services. HomeBase provides the following services directly or through referrals to our community partners:

• Budgeting and financial workshops

• Case management services

• Education and employment services

• Entitlement advocacy

• Legal services/informal mediation

• Resources for how to look for housing

• Short-term financial assistance

HomeBase is also an aftercare provider to those who have left the shelter with a subsidy and those placed in NYCHA housing.

This program is funded by the NYC Department of Homeless Services. CAMBA’s mobile You Can Van was piloted by the Durst Family Foundation, and expanded to high-risk communities in Staten Island by the Robin Hood Foundation.

Our Therapeutic Supervised Visitation Program works with families to promote safe, healthy and consistent parent-child relationships in the aftermath of family trauma and separation. Highly skilled licensed therapists strive to help custodial and visiting parents to better understand and meet the physical and emotional needs of their children through psychoeducation, skill building, and processing the impact of family trauma and separation. The program is child-centered, strengths-based, and trauma-focused.

NYCHA West Brighton Houses could be selected to undergo more than $100M in repairs

STATEN ISLAND, N.Y. -- The New York City Housing Authority (NYCHA) West Brighton I and II complex will soon undergo a transformative renovation expected to vastly improve the quality of life of the apartments’ nearly 1,500 residents.

The massive renovation project, announced last year , will address more than $100 million in capital repairs needs, and include a complete overhaul of the complex’s apartments, building infrastructure and common areas.

All apartments will receive new kitchens, bathrooms, flooring, windows, paint, doors and more, while infrastructure upgrades include enhancements to building security, heating and cooling, water and plumbing and ventilation. The project will also improve public spaces, community safety, landscaping and building entryways, according to NYCHA.

Although design work on the project remains in progress, housing authority officials told the Staten Island Advance/SILive.com that construction is expected to begin this summer. However, NYCHA couldn’t yet provide a specific start date.

The project team has been working closely with residents on the design process, including months of in-person and virtual meetings, in-home assessments, community events, surveying, model unit tours and other resident input activities.

Once the design has been completed, it will be outlined in the West Brighton Community Plan, which will be posted online and distributed to all residents before construction begins.

During construction, some residents will be temporarily relocated to other units on-site to accommodate elevator modernization work and the remediation of environmental hazards within their units.

All residents will be returned to their original units once the work is completed.

City officials said construction is expected to take approximately 30 months, or two and a half years, to be completed, putting the project on track to wrap up in 2026 or 2027.

ABOUT THE PROJECT

The project will be completed through NYCHA’s Permanent Affordability Committment Together (PACT) program, which is the agency’s version of the Rental Assistance Demonstration (RAD), a U.S. Dept. of Housing and Urban Development (HUD) program created for housing authorities to access federal capital to improve developments and ensure their affordability.

Under PACT, NYCHA establishes public-private partnerships with private developers, property management companies and social service providers, to carry out much-needed overhaul renovations of their developments. They also convert the properties from Section 9 (public housing) to project-based Section 8, the Housing Choice Voucher program.

NYCHA remains the owner of the buildings, but the PACT program allows day-to-day management of the complexes to be handed over to private companies. Pinnacle City Living will serve as West Brighton I and II’s property manager after they are converted, according to NYCHA.

BFC Partners Development LLC, CB Emmanuel Realty LLC and non-profit organization Catholic Homes New York will work together to repair 574 apartments across the NYCHA development’s 14 buildings, which are home to more than 1,400 residents.

NYCHA and residents of West Brighton I and II, with the support of a technical adviser, Censere Consulting, underwent an extensive community engagement process that began in fall 2021. The committee then reviewed proposals from several potential partners and conducted interviews before selecting BFC Partners, CB Emmanuel Realty and Catholic Homes New York.

“West Brighton Houses is in desperate need and is long overdue for these comprehensive repairs provided by the PACT program,” Shekina Butler, president of the West Brighton Tenant Association said last year. “The quality of life for West Brighton’s residents has declined over the years, and now it’s time to celebrate the upcoming changes. While this has been a long and arduous process, we are thankful for the support of NYCHA and Censere Consulting to have finally chosen the partner team that we feel will best suit the needs of West Brighton residents.”

NYCHA is seven years into its efforts to upgrade its developments, many of which have not been renovated in decades. According to the agency’s website , more than 20,000 apartment units have received repairs or are in progress under the PACT program. There are currently 72 active PACT projects.

New York lawmakers have toughened laws in a bid to attack the problem of squatters who illegally occupy houses.

But homeowners can also proactively protect themselves from crafty squatters, who have plenty of weapons at their disposal when it comes to stealing a home and claiming it as their own.

TOUGHENING SQUATTER LAWS

Language in the recently passed state budget excludes squatters from tenant protections.

An earlier bill from state Sen. Jessica Scarcella-Spanton (D-North Shore/Southern Brooklyn)  contained similar squatter-defining language; doubled the time period for tenancy rights from 30 to 60 days of possession; incorporated squatting into the definition of criminal trespass in the third degree, and enhanced lease provisions.

“For too long, my constituents have grappled with the current reality where individuals unlawfully occupy their spaces without any recourse. We witness distressing scenarios unfold in the news week after week — hardworking homeowners facing legal repercussions for rightfully reclaiming their own property from individuals who brazenly occupy it without consent,” Scarcella-Spanton said. “No one should return home to find their property seized by squatters, who now have more rights to their property than they do.”
