10 travel companies for eco-conscious travellers
By Juliet Kinsman
Conscious consumers need quick signposts to more responsible spending. A company displaying its B Corp status is welcome short-hand for telling us that the company has gone through a rigorous assessment of its impact on workers, customers, suppliers, community and the environment.
‘As the travel industry, and all businesses, seek to recover from the effects of the pandemic, the sector is evolving to offer more sustainable ways of exploring the world – and B Corps can play a crucial part in accelerating this trend,’ says Chris Turner, executive director of B Lab UK , which certifies B Corp businesses. ‘Increasingly, travel ventures are taking important and necessary steps to mitigate their impact on the environment. For B Corps, creating a positive impact means going beyond protecting our planet to empower workers, serve communities and legally embed these principles at the heart of the business.’
What’s the big deal about looking out for this marque on a website or products? Any business actively aiming to minimise harm to the environment and maximise human welfare every step of the way is naturally going to be more appealing to those striving to make more responsible purchases and live a more sustainable lifestyle.
B Corporation holiday heroes
Travel companies aiming to be more sustainable are increasingly seeking that B Corp seal of approval and lots more businesses are applying for the accreditation since the first few dozen went live in 2007. To qualify, a minimum of 80 points out of 200 must be scored on the B Impact Score Assessment.
Sawday’s, UK
The first accommodation platform in the UK to achieve B Corp accreditation, Bristol-based Sawday’s group (which includes glamping directory Canopy & Stars and pet holiday company Paws & Stay ), was founded on principles of social and environmental responsibility. The company is structured in such a way that it's been majority employee-owned since 2018, with a quarter owned by a charitable trust. It aims to plant a million trees by 2025 as part of a drive towards being a carbon-neutral office, plus it ranks high for staff wellbeing and happiness and for making an impressive contribution to local communities. ‘We are at a critical point for the environment and coming out of the pandemic, we have a once in a lifetime chance to reinvent responsible and ethical holidays – we must encourage everyone to think differently,’ says Mike Bevan. The company is also a member of Travel by B Corp , a growing collection of UK-based B Corp travel businesses. sawdays.co.uk
A small, bespoke, high-end travel designer set up in 2017, specialising in personalised itineraries. Joro tries to make travelling more sustainable for its clients by communicating the carbon footprint implications of every step, prioritising electric vehicles for transfers, direct flights and the right accommodation choices. It has also founded the Conscious Travel Foundation to support conservation and community projects and to drive change. joroexperiences.com
Journeys with Purpose, UK
Score 129.8.
Founded with conservation at its core, Duncan Grossart’s agency plans nature-positive adventures. Each trip is planned around a specific intention, often rewilding . ‘Rewilding is the large-scale, long-term restoration of ecosystems where nature is allowed to take care of itself,’ explains the founder and zoologist. 'Restoring biodiversity is a key pillar in the building back of our planet’s diminishing life systems, and rewilding apex predators and keystone species is a fundamental aspect of this process.
journeyswithpurpose.org
Intrepid, worldwide
The world’s largest adventure travel company, which has its headquarters in Australia , was the first global travel firm to achieve B Corp status. James Thornton, its CEO, recognises that although there are hundreds of certification schemes in travel, few take a holistic view of impact in the way that B Corp does. ‘It took us three years to become certified, a painstaking process that has fundamentally changed the way we do business,’ he says. ‘But having that certification means that people who travel with us trust that we’re fully committed to benefitting people and the planet.’ intrepidtravel.com
This slow travel holiday operator makes complicated multi-stop journeys by train, bus and boat seem a breeze. By making it easier to eschew air travel, Byway plots flight-free itineraries booking locally-owned accommodation along the way. The British brand born during the pandemic is also introducing carbon labelling on their trips. Founder Cat Jones says, ‘We love that our B Corp status shows we’re up for being entirely transparent about, and accountable for our progress towards, our social and environmental goals. These include reducing the climate impact of leisure travel through our 100 per cent flight-free holidays and taking tourists away from hotpots to wonderful places off from the beaten path.' Byway is also prioritising delightful lesser-known locations over tourist magnets, and choosing lower-carbon routes such as the Scotrail service to cross the Glenfinnan viaduct in Scotland over the famous Jacobite Express steam train that runs on the same route.

Pura Aventura, UK
Spain , Portugal and Latin America await via this Brighton -based travel operator. It also measures the carbon footprint of your entire foray, which is offset through reforesting in the poorest part of Nicaragua .
‘Being B Corp means being one of a diverse group of companies which is greater than the sum of its parts. Large and small, we are all driven by a desire to have a positive impact,’ enthuses founder Thomas Power. ‘When we work together on a problem, narrow self-interest is left at the door and the world gets a turbo-charged collective of change-makers. B Corp gives you hope.’ pura-aventura.com
Amazonas Explorer
‘The concept of putting people and planet before profit is the future of how we should all be doing business,’ says Paul Cripps, managing director of this Peruvian travel company. ‘It is not easy to become a B Corp but so worthwhile. Every year we strive to get better, especially as you need to recertify every three years with a higher score – it’s not a one-off certification, but an evolving one.’ Having arranged Machu Picchu-pegged itineraries for three decades, Amazonas aims to work only with like-minded hotels and suppliers, and it’s taking all its tours carbon neutral by offsetting into a carbon sink in Manú National Park in southern Peru. amazonas-explorer.com
Blue Apple Beach, Colombia
This boutique hotel and beach club on the island of Tierra Bomba near Cartagena is a purpose-driven company from Portia Hart. Born to a Trinidadian mother and a British father and raised in rural England , Hart was instilled from an early age with the importance of social equality, as well as being in balance with nature. With this B-Corp recognition, Blue Apple Beach joins only 73 certified Colombian companies to have achieved the certification. On a greater scale, Blue Apple Beach’s B-Corp status proves that regardless of the size of your business, it is possible to meaningfully implement positive practices and become a changemaker in your industry. It is further evidence of how conscious consumers rely on these certifications as they research businesses, impacting their decisions when spending.
blueapplebeach.com
Bodhi Surf + Yoga, Costa Rica
Score 121.5.
Responsible practices have been woven into every aspect of this lodge in Costa Rica since it opened in 2010. Keen to have this validated by a third party, it’s the only surf camp or yoga retreat with the B Corp seal of approval – and it’s keen for others to catch up. Bodhi's Service and Surf programme in the Ballena Marine National Park shares knowledge and resources with community members, especially the young, in exchange for community service and beach clean-ups. bodhisurfyoga.com
International Volunteer HQ, New Zealand
The first and still the only global volunteer and internship travel specialist to become a certified B Corporation, this New Zealand-based organisation offers volunteering projects and international internships in more than 50 destinations, focusing on delivering impact for local communities, wildlife and the environment. All its projects have been developed to support the UN's Sustainable Development Goals and in the last year, it has launched more volunteer projects to support climate action. Its business operations have been carbon neutral since 2019 and its focus on sustainability and regenerative travel has seen the company increase its B Impact Score in each audit undertaken since 2015. volunteerhq.org

Citi is an advertising partner.
Citi has rebranded its popular mid-tier Citi Premier® travel credit card as the Citi Strata Premier ℠ Card . The new Strata Premier Card is nearly identical to the old Citi Premier except it has additional travel protections, bonus spending categories and a new limited-time welcome offer .

Citi Strata Premier℠ Card details
The Citi Strata Premier ℠ Card earns transferrable Citi ThankYou points and earns them in bunches. It also currently offers a welcome bonus worth 70,000 points after spending $4,000 in the first three months of account opening.
This bonus is worth $700 in gift cards, cash back or travel (booked through thankyou.com ). To potentially get even greater value, you can also transfer these points to any Citi ThankYou travel partners , including Air France/KLM (Flying Blue), Avianca, Singapore Airlines and Virgin Atlantic. Having the option to transfer your points unlocks all sorts of opportunities.
For example, you could take advantage of the monthly Flying Blue promo rewards and use the bonus points to book two round-trip economy awards flights from the U.S. to Europe which can easily cost over $500 a person depending on your itinerary. Want to go somewhere a little more tropical? Transfer those 70,000 points to Avianca LifeMiles and you have enough to book two round-trip economy awards flights from the U.S. to the Caribbean .
New Citi Strata Premier Card benefits
Aside from a new name and some travel protections, the new Citi Strata Premier is mostly the same as the Premier card it replaces.
One change is that the Strata Premier is making permanent the old Premier's limited-time spending category bonus of 10X points on hotels, car rentals and attractions when booked through CitiTravel.com You're also now able to earn 3X at EV charging stations.
But the most notable changes with the Strata Premier are the new travel coverages, including:
- Trip cancellation/interruption
- Lost or damaged luggage
- MasterRental coverage (car rental insurance)
What's staying the same with the Citi Strata Premier Card?
The Citi Strata Premier's annual fee is staying at $95, and you'll continue to earn:
- 3X points on air travel
- 3X points on hotels (not booked through CitiTravel.com)
- 3X points at restaurants
- 3X points at supermarkets
- 3X points at gas stations
- 1X points on all other purchases
You'll still receive an annual hotel benefit valid for $100 off a single hotel stay of $500 or more that you book through CitiTravel.com. This card continues to have no foreign transaction fees and access to Citi Entertainment as well as all of the same Citi ThankYou transfer partners.
As before, if you have a Citi cash-back card, such as the Citi Double Cash® Card or the Citi Custom Cash® Card , you can pair it with the Citi Strata to convert your cash-back rewards into fully transferrable points.
Citi Double Cash® Card
Earn 2% on every purchase with unlimited 1% cash back when you buy, plus an additional 1% as you pay for those purchases. To earn cash back, pay at least the minimum due on time. How to qualify for the Citi Strata Premier Card
You'll want a good-to- excellent credit score to have the best chance of being approved for the Citi Strata Premier. There is also an additional restriction for qualifying for this card's intro bonus. If you've received an intro bonus for the Strata Premier or the Premier card within the last 48 months, you can't earn the bonus again with a new Strata Premier Card.
It's important to note that this 48-month restriction applies to when you received the intro bonus points, not when you opened the account. If you opened a Citi Premier card in April of 2020 but didn't earn the intro bonus until early July 2020, you have to wait 48 months from July — not April — to be able to earn the Strata Premier's bonus.
Alternatives to the Citi Strata Premier Card
The Chase Sapphire Preferred® Card charges the same annual fee as the Strata Premier Card and offers similar benefits. It too earns transferrable travel rewards and has a long list of travel and purchase protections. But while the cards seem similar on the surface, dig a bit deeper and you'll find the Chase Sapphire Preferred Card has a few advantages over the Citi card.
The Chase Ultimate Rewards® airline and hotel transfer partners include programs such as World of Hyatt, Southwest Rapid Rewards and Air Canada Aeroplan, which help make Chase's network one of the best in the travel rewards space. The Sapphire Preferred also features more robust protections than the Strata Premier, including purchase protection and extended warranty protection on top of travel insurance.
Chase Sapphire Preferred® Card
Enjoy benefits such as 5x on travel purchased through Chase Travel℠, 3x on dining, select streaming services and online groceries, 2x on all other travel purchases, 1x on all other purchases, and $50 annual Chase Travel Hotel Credit, plus more.
Earn 75,000 bonus points after you spend $4,000 on purchases in the first 3 months from account opening. That's over $900 when you redeem through Chase Travel℠.

The Capital One Venture Rewards Credit Card (see rates and fees ) doesn't have the same long list of bonus spending categories you get with the Strata Premier. Instead, it earns at least 2X miles on every purchase, which makes it a strong option for everyday purchases outside of standard bonus categories. The Capital One Venture Rewards also allows you to transfer miles to a strong list of partner travel programs or keep things simple by offsetting travel purchases with miles at a rate of one cent each.
Capital One Venture Rewards Credit Card
5 Miles per dollar on hotel and rental cars booked through Capital One Travel, 2X miles per dollar on every other purchase
Earn 75,000 bonus miles once you spend $4,000 on purchases within 3 months from account opening
N/A for purchases and balance transfers
19.99% - 29.99% variable
$0 at the Transfer APR, 4% of the amount of each transferred balance that posts to your account at a promotional APR that Capital One may offer to you
Bottom line
The Citi Premier Card is now the Citi Strata Premier . With this renaming, a few of the benefits are changing, but it is largely the same card. However, the biggest weakness of the Citi Premier has been solved with this update, which includes the addition of travel insurance perks.

What to look for in a Travel Management Company?
Finding the right Travel Management Company (TMC) to partner with your organization requires strategic planning, a methodical process and involvement from the right travel stakeholders.
Travel buyers are responsible for how an organization uses travel and meetings to help achieve their broader goals. Partnering with the right TMC leads to improved safety, productivity and wellbeing for business travelers and streamlined processes and cost savings for your organization.
The TMC should demonstrate broad experience in managing travel for organizations like yours. Identify the specific services you need from them, such as ability to connect travel program strategy to company mission and purpose, sector experience, hotel program expertise, sustainability, travel policy compliance, risk management, or a defined approach to Diversity, Equity and Inclusion (DE&I).

Why Is Car Insurance So Expensive?
Soaring premiums have become a prominent driver of inflation, and insurers say that more increases could be on the way. How did it get like this?
- Share full article
By Emily Flitter
If your car broke down two years ago, it probably became a bigger problem than you bargained for.
A confluence of forces were to blame: The Covid pandemic disrupted supply chains, pushing used car prices to record highs and making spare parts hard to get; out-of-practice drivers emerging from lockdowns caused more severe wrecks; and technological advancements like motion sensors made even the simplest parts, like a fender or a rim, expensive to replace .
Things have since improved for car owners — except when it comes to insurance bills. Car insurers are still raising prices steeply: The price of motor vehicle insurance rose more than 22 percent in the year through April, the fastest pace since the 1970s, according to a report the Bureau of Labor Statistics on Wednesday. According to calculations by the Insurance Information Institute, a trade group, the average 12-month premium for car insurance was $1,280 in 2023, the industry’s most recent figures.
That has made car insurance a prominent factor preventing overall inflation from cooling more quickly, which could force the Federal Reserve to keep interest rates higher for longer even as the prices for many other essential goods and services have slowed.
Geico recently reported a big jump in quarterly profit on higher premiums and lower customer claims. The share prices of other big auto insurers, like Allstate and Progressive, have beaten the rise in the overall market this year.
That has attracted scrutiny from economists . A key reason car insurance costs are rising so fast right now has to do with how the industry is regulated.
How does insurance regulation work?
Insurers are regulated by the states, not the federal government. In all 50 states, insurance companies must follow specific rules about how and when they can raise the price on their policies.
Each state’s laws are broadly similar, and require insurers to ask regulators for permission to raise prices. Insurers have to make a case — with data to back it up — that the increase is necessary and that they will not make too large a profit on the re-priced policies. This application, known in the business as a “rate filing,” involves complicated paperwork that may take weeks or months to resolve.
The data has to include an analysis of loss trends from the past couple of years, as well as projections for replacement costs and profits. If insurers are deemed to profit too heavily, regulators can make them return money to customers.
The threat of returning money is not an idle one. At the height of pandemic lockdowns in 2020, when many cars sat idle, insurers returned almost $13 billion to customers through dividends, refund checks and premium reductions for policy renewals, according to the insurance ratings agency AM Best.
California was one of the most active states: Insurers there returned $3.2 billion to customers in 2020.
Ricardo Lara, the state’s insurance commissioner, “directed the department to do a very close analysis to make sure that drivers weren’t overcharged,” said Michael Soller, a spokesman for the California Department of Insurance. But starting in late 2021, the state became the poster child for a new problem: an epic backlog of insurers’ requests to raise prices.
How a massive paperwork jam explains rising prices.
When the pandemic shut down most economic activity, it messed up insurers’ ability to use the past to predict the future. For months, they were frozen. They did not submit new rate filings to regulators for a spell — until they did, all at once, in the second half of 2021.
The prices of cars and parts were jumping and drivers were back on the roads and crashing left and right after a hiatus behind the wheel.
“You went from this period of incredible profitability to incredible losses in the blink of an eye,” said Tim Zawacki, an analyst who focuses on insurance at S&P Global Market Intelligence. No companies were willing to stick their necks out by offering lower premiums in the hope of winning new business, he said.
“Everyone was together in significantly pushing for rate increases.”
In California, the most populous U.S. state, insurers were getting creamed by expensive claims.
But the state’s regulator did not start approving insurers’ requests to raise rates until near the end of 2022. The backlog grew so large that the average wait time for approvals was longer — by several months — than the six-month policies that insurers wanted to sell.
“When state regulators delay or prevent companies from accurately pricing insurance, insurers may not be able to absorb the costs,” said Neil Alldredge, the president of the National Association of Mutual Insurance Companies, a trade group that represents many home and auto insurers. The squeeze can lead insurers to leave some states or stop some business lines, he added. “Inefficient regulatory environments in states like California, New Jersey and New York, combined with inflation and increased catastrophic losses, have left consumers with fewer choices of insurers and higher costs,” he said.
California is still the slowest state in the continental United States for auto insurance rate filings, taking an average of 219 days to approve a price proposal for a personal auto policy, according to S&P data provided by Mr. Zawacki.
“We fight for consumers by analyzing all of the data, not just what insurance companies spoon-feed us,” Mr. Soller, the California Department of Insurance spokesman, said.
The S&P analysis showed that New Jersey, the 11th-most populous state, had the sixth-longest wait time, while New York, with the fourth-largest population, had the 7th-longest wait times.
“The department performs a comprehensive review of requests to amend rates or rating systems to ensure compliance with New Jersey law,” said Dawn Thomas, a spokeswoman for the New Jersey Department of Banking and Insurance.
Ms. Thomas said the regulator needed to ensure that each proposed premium increase was “reasonable, adequate, and not unfairly discriminatory,” and that sometimes the insurers’ requests needed to be challenged or denied.
A spokeswoman for New York’s regulator declined to comment.
When will the jam clear?
Shortly before the pandemic, the umbrella organization for state insurance regulators, the National Association of Insurance Commissioners, formed a team of data scientists to help regulators deal with their rate filings, which has gotten more complicated in recent years.
The data team became fully operational in 2021 and its mission is now to help speed up the review process: 37 states have signed up to use it.
This month, during a call with analysts to discuss Allstate’s earnings, company representatives said they had recently reopened their California auto insurance business after getting permission to charge higher rates. The company still wanted to raise prices in other states.
In New York and New Jersey, for example, “even with the rate approvals that we got late last year, we still don’t feel like we’re at the appropriate rate level to want to grow in those two states,” said Mario Rizzo, the president of Allstate’s property-casualty business.
How much higher will premiums go?
In 2021, insurers’ personal auto businesses started recording losses. According to David Blades, an analyst for AM Best, the industry lost $4 billion in 2021, $33 billion in 2022 and roughly $17 billion last year.
According to Dale Porfilio, the chief insurance officer at the Insurance Information Institute, the trade group, many companies still need to raise prices to make up for those bad years.
Last year, insurers raised auto premiums by 14 percent, the biggest increase in over 15 years. Mr. Porfilio’s best guess is that premiums this year will rise another 13 percent.
“It’s going to take time for every company to get their rates to where they want to be,” he said.

IR-2024-133, May 6, 2024
WASHINGTON ― The Internal Revenue Service today reminded the 1 million people who didn’t file their tax year 2020 returns they may be eligible for a refund if they file by the May 17 deadline.
The IRS estimates more than $1 billion in refunds remain unclaimed because people haven’t filed their 2020 tax returns yet. The state-by-state table below shows how many people are potentially eligible for these refunds, and the average median refund in each state.
There's no penalty for failure to file if a refund is due. However, a return claiming a refund must be filed within three years of its due date for a refund to be allowed. After the expiration of the three-year period, the refund statute generally prevents the issuance of a refund check and the application of any credits, including overpayments of estimated taxes or withholding amounts, to other tax years that are underpaid.
For 2020 tax returns, people have a little more time than usual to file their claim for refunds. Typically, the filing deadline to claim old refunds falls around the tax deadline of April 15. However, the 2020 filing deadline was pushed to May 17, due to COVID-19, making the three-year window deadline for 2020 unfiled returns May 17, 2024. The IRS issued Notice 2023-21 on Feb. 27, 2023, providing legal guidance on claims required by the postponed deadline.
Many people who didn’t file may be eligible for more
By missing out on filing a tax return, people stand to lose more than just their refund of taxes withheld or paid during 2020. The estimate does not include credits that non-filers may be eligible to receive. Credits include the Earned Income Tax Credit (EITC) , the Recovery Rebate Credit or other credits that may be applicable.
Many low- and moderate-income workers may be eligible for the Earned Income Tax Credit (EITC). For 2020, the EITC was worth as much as $6,660 for taxpayers with qualifying children. The EITC helps individuals and families whose incomes are below certain thresholds.
The IRS has previously reminded those who may be entitled to the COVID-era Recovery Rebate Credit in 2020 that time is running out to file a tax return and claim their money. The Recovery Rebate Credit is a refundable credit for individuals who did not receive one or more Economic Impact Payments, also known as stimulus payments, distributed in 2020 and 2021.
Plan to file? IRS offers options to get key documents
Gathering all the necessary documents and forms to file a return for 2020 may take some time. People should start as soon as possible to make sure they have enough time to file before the May 17 deadline for 2020 refunds. Here are some options:
- Request copies of key documents. Taxpayers who are missing Forms W-2, 1098, 1099 or 5498 for the years 2020, 2021 or 2022 can request copies from their employer, bank or other payers.
- Use Get Transcript Online at IRS.gov. Taxpayers who have lost contact with their employer or other payers can order a free wage and income transcript at IRS.gov using the Get Transcript Online tool. For many taxpayers, this is by far the quickest and easiest option.
- Request a transcript. Another option is for people to file Form 4506-T, Request for Transcript of Tax Return , with the IRS to request a “wage and income transcript.” A wage and income transcript shows data from information returns received by the IRS, such as Forms W-2, 1099, 1098, Form 5498 and IRA contribution information. Taxpayers can use the information from the transcript to file their tax return. But plan ahead – these written requests can take several weeks; people are strongly urged to try the other options first.
- Current and prior years’ tax forms. Tax year 2020 Forms 1040 and 1040-SR and instructions are available on the IRS.gov Forms, instructions & publications page or by calling toll-free 800-TAX-FORM (800-829-3676).
Taxpayers who are unsure if they are required to file a return can visit Do I need to file a tax return? or refer to Publication 17, Your Federal Income Tax (For Individuals) .
Free help is available
For individuals who have not filed a federal income a tax return for 2020, the IRS offers tools and resources on IRS.gov. Free support includes the Interactive Tax Assistant (ITA) , information on What to do if you haven’t filed your return , and Frequently asked questions and answers (FAQs) .
Qualified taxpayers can also access free tax preparation assistance through the Volunteer Income Tax Assistance and the Tax Counseling for the Elderly programs . Use the VITA Locator Tool or call 800-906-9887 to locate the nearest VITA site.
The IRS also reassures taxpayers there is no penalty for claiming a refund on a late-filed tax return. Direct deposit is recommended as the quickest and simplest way to receive a tax refund.
State-by-state estimates of individuals who may be due 2020 income tax refunds
The IRS estimated the number of individuals in each state and the median potential refund a filer may be entitled to receive. The actual refund amount will vary based on a household’s tax situation.

Families forced to wait up to three years to see paediatric specialists, or pay hefty costs for city appointments
Jack Anderson is about to turn four. Like his little brother Ripley, he was born prematurely.
For three years Jack was on a waiting list to be assessed for autism and ADHD, conditions for which, according to Autism Awareness Australia, early intervention and access to timely, specialised support is crucial.
His mother, Maddison Wood, said Jack's occupational therapist had "pushed" for the assessment, which led to his eventual diagnosis, since he was about eight months old.
Ripley, who is almost one, was born with kidney and pancreatic abnormalities, as well as a severely clubbed foot.
Ms Wood said she had spent around $50,000 in the past year mainly on Ripley's health needs, including travel to Adelaide to visit paediatric specialists not available in her home town Mount Gambier.
While long waiting lists and limited access to paediatric specialists is not unique to rural South Australia — there's a shortage of paediatricians nationwide — it is cold comfort for families told of the importance of early intervention only to be unable to access it.
Rural Doctors Association of SA president Bill Geyer said there were "a number of things at play" when it came to better access to paediatric services in rural areas, but top of the list was the shortage of resident specialists.
"The state government had a think-tank in December 2022 looking at this issue, and obstetricians and paediatricians were the two specialists shown to be most lacking in rural areas," he said.
"The Rural Doctors Workforce Agency provide fly-in fly-out specialists travelling across the state, but waitlists are substantial."
Dr Geyer said the industry was improving rotations for trainee specialists to go to country areas with the hope they might get a taste for rural life and choose to stay.
"Fly-in fly-out services are better than nothing, but you can't beat having resident specialists who provide continuity of care."
Shortage bites nationwide
According to the Royal Australasian College of Physicians (RACP), there is no shortage of interest in specialist paediatric positions; it says the limiting factor is available government funding to expand specialist training programs.
"Unfortunately there remains a significant shortage of non-GP specialists in many rural and regional areas," president Jacqueline Small said.
Dr Small said the RACP administered about 380 specialist training program positions across Australia, but "this number of places is unlikely to be enough to address the under-resourcing in regional areas".
In 2022, it was revealed in the West Australian parliament that children were waiting up to 16 months to see a paediatrician in the public health system, and 10 months for an appointment with a clinical psychologist.
Research by Perth paediatrician Lana Bell found that a third of private paediatricians and private child psychiatrists had closed their books completely, and another third had waitlists of more than six months.
The findings prompted the federal government to commit to increasing the number of GPs and allied healthcare workers in regional areas, but Ms Wood said there was a significant gap in specialist services.
"The specialists aren't here, or if they do fly in, it's almost impossible to get an appointment."
She said her family travelled to Adelaide "every two or three weeks" to access specialist paediatric services for the boys.
"Being a parent, you know when something is wrong with your child but trying to get them diagnosed is a long, long road.
"We've encountered 12 to 24-month waitlists to get into a paediatrician.
"If you can get in to see an [occupational therapist] or a different type of specialist, sometimes they will push for you to get the appointment with a paediatrician, otherwise you're on your own and you have to keep calling and calling.
"Waiting almost three years for an assessment is crazy."
500-kilometre round trips
Coonawarra mother Khloe McElroy is experiencing similar long wait times to get support for her son Chace, who was also born prematurely.
In Mount Gambier, a city of about 28,000 people, there is just one resident paediatrician, who is supported by locums.
"Chace has ongoing health issues, recurring illnesses and is in hospital in Mount Gambier every two to three weeks," Ms McElroy said.
She said they had tried to find a paediatrician and started sending out referrals in February; after sending about 10 through their GP they were able to see one in April.
"Everyone kept declining. We even tried to go private and still couldn't get an appointment until a paediatrician in Warrnambool [across the border in Victoria] sent a direct letter stating that it is not OK to refuse treatment for a patient who is failing to thrive.
"[They said] if the local paediatricians are doing that, then the medical board needs to be contacted.
"We finally did get to see a paediatrician in Mount Gambier but then the next available appointment was another three-month wait. It's extremely frustrating and heartbreaking."
She said getting an appointment seemed to be a case of being the loudest voice and continuing to call, sometimes daily, for appointments.
"But if you don't advocate for your child, who will?"
Employers unable to attract, retain specialists
Heather Cary, who runs speech pathology clinic Communicate Better in Mount Gambier, said there was a nationwide shortage of professionals in her industry, but the situation was "far worse" in rural and regional areas.
"Unfortunately I have an extensive waitlist — children are waiting two to three years for speech pathology support," she said.
Ms Cary has two allied health assistants in her practice and more than 30 clients ranging from babies to teenagers.
"I have tried to recruit more speech pathologists but they're just not in the community.
"It's very frustrating and sad because communication is a basic human right and children should not be waiting for these services.
"The impacts of delayed support can cause further gaps in the child's communication development, but it can also cause a range of behaviours as well because the child is so frustrated being unable to communicate.
"I will often tell families to get on every waitlist in town or trial telehealth services so they can get some support in place, but even that is tricky because there's nationwide shortages of speech pathologists."
Specialists needed as cities grow
Jessie Bilal, the practice manager at Mount Gambier's Village Medical Centre, speaks to hundreds of families trying to access specialist medical care.
"It can be really heartbreaking when patients need to access specialist services which are not available locally."
She said families really had only two choices:
- to travel with minimal reimbursements from PATS (Patients Assistance Transport Scheme), placing them under financial stress and isolating them from family and support networks; or
- to be placed on lengthy waiting lists for an appointment closer to home.
Ms Bilal said regional areas were in dire need of funding for additional services, which included enticing specialists to live or travel to regional areas.
"The city has grown so much post COVID, and our health infrastructure needs an entire overhaul to be able to sustain our community's needs as it continues to grow," she said.
Families choose to live apart
Mount Gambier mother of two Eva Kovacs said she and her partner made the difficult decision to separate their living arrangements to accommodate their children's needs.
"The boys, born with the rare genetic disorder Prader-Willi syndrome, now live with their father in Geelong in Victoria so they can access specialist care," said Ms Kovacs, who remains in Mount Gambier for work and other family commitments.
She said she saw her boys during school holidays, "which isn't ideal".
"They aren't always able to travel back to Mount Gambier, and the car trip is hard on them, so I go to them or we meet half way.
"If we could access medical care and regular specialist support locally, more families could stay together."
